Staff Reviews
We pondered quite a bit about how to rate using our Christmas Report Card system.  We feel that the staff scores might best not be combined with the doctor’s score.  With high performing doctors that staff more often than not drives their bosses scores down.  Some offices take on interns and work-study people.  If these trainees make an error in practice we feel it should not effect the primaries score.  For the first time this year we separate out staff scores for three offices: Boyce, Nguyen, and Mojica.  The titles for our staff assessments are: Ophthalmology 2016 - TQN 2016 Staff - CM 2016 Staff.

Doctor Boyce’s Tech was proud enough to state her name (Martha) and her role at the beginning of the appointment.  We find that this is usual and customary.  TQN’s Tech neither stated her name nor her role.  An anonymous dermatology tech stated that she was going to do things different – she really didn’t deliver on this promise.  The unidentified dermatology tech scores lowest on courtesy and bedside manner.  The results of her freeze times are pending.  It would be nice if she shared knowledge by describing the lesions using the proper terms.  We have a lot of harmless “old age spots”.

Introducing
We here at MSG are developing our HealthMan product bundle for release in 2017.  This is the pre-sell for HealthMan and HealthMan Utilities.

Vitals V Healthgrades Format
All things being equal we would use Healthgrades Method more and more.  For our in-house ratings such as MSG’s Christmas Card Report (CRC) we are using Healthgrades Methods for a standard foundation to build upon.  When we need objective information about Rating Your Doctor (RYD) we collect up as much as we can from other sites such as Vitals.


HealthMan Applied - PCP & AnnoyMan HEDIS Driven Fitness Score (HDFS)
One of the utilities in our HealthMan Bundle is AnnoyMan.  LiveX – After completing our yearly (HEDIS Prescribed) physical we reflect.  Our PCP only pointed out our defects.  So we begin addressing what we call a HEDIS Driven Fitness Score (HDFS).

Array – Baseline Results for MSG-HDFA V 2016-19 Items Beta (The Client “Us”) – HEDIS Derived Fitness Score (HDFS)

Purpose (AnnoyMan)
MSG’s HEDIS Driven Fitness Assessment was designed to address deficits pointed out by our primary care physician (PCP) at our last (2016) HEDIS driven physical.  He addressed only our deficits.  We thought wow!  We are doing pretty well.  We informally observe about ten (10) of his other patients.  Relative to them we appear to be doing pretty well.  Here we share the results of our Beta version HEDIS Driven Fitness Assessment (HDFA).  This version treats the some odd nineteen (19) HEDIS elements with equal weight.  Over the course of 2017 we will refine it a bit.  For every deficit such as diabetes or COPD a PCP might balance with an obvious strength such as a non-pot smoker, delivering a clean Substances Of Abuse (SOA) urine test or not being dependent on an inhaler.  The physical was annoying.

Feature - How Healthgrades Works
Healthgrades - Wikipedia

Basically, Healthgrades appears to be an on-line registry of doctors and other health care practitioners.  Healthgrades permits consumers to rate professionals online.  There are other sites to Rate Your Doctor such as Vitals.  Healthgrades commercial-of-the-shelf rating tool(s) are more comprehensive than Vitals.  The most important thing we have taken note of was that the Healthgrades system to share ratings in Orange County California is underutilized.  Most doctors have less than ten (10) submissions.  One of our reference doctors, Boyce, has the most online ratings for our cohort of health professionals.  Hint rate your doctors and staff so the score base approach numbers significance.

Giving In-Kind
Part of our Attitude Of Gratitude (AOG) spiritual approach is our year Giving-In-Kind (GIK) method. We chartered our gifts-in-kind back on April 4, 2011.  We sought to repay those that helped us in times of hardship.  Yet most of our helpers didn’t and do not need help.  For example we offer moving services to those that help us move.  Yet so far none of our mover-helpers have needed to move. For example my aunt (B 1925) refuses to let us modularize her garage in preparation for her retirement and moving.  This year we are not offering formal invitations for giving it back.  The GIK effort was transferred to this Christmas Report Card Project.

Attitude Of Gratitude (AOG) – Spiritual Principle.
What we were grateful for in 2016 is far reaching.  Across all psychosocial domains we have achieved net gain this year.  In a couple of domains we have hit it out of the park.  One of our strongest domains is Technological Advance.

Rate Your Doctor
Why rate your doctor?  We do it to help them measure their performance with the intent to offer a baseline from which to work.  We hope that we help them with accountability, transparency, and digital survivability.  We just wish to help them move from practicing to actual delivering tangible results.

Promising Practice
For us this Christmas Report Card looks like a promising practice.

Assessments
The assessments included here are of the following types.  We have quick score method (QSM) assessments, commercial-off-the-shelf assessments, screening type assessments, fleshing assessments, and exhaustive assessments.  The assessments included here were either pirated, modified or derived.

Normal Distributions
Normal distribution may be defined as a function that represents the distribution of many random variables as a symmetrical bell-shaped graph.  For the most part our random variables are our satisfaction and performance scores for individuals.  Our distribution is a bit skewed towards the right or favorable because we are focusing on gratitude.

CMS Stars
The program that is at the center of most of this study is Brand New Day.  When we were first enrolled 2012, BND shared their CMS Stars score with our enrollment materials.  In 2012 CMS awarded BND 3.0 Stars.  In 2016 CMS again awarded BND 3.0 Stars.  Unfortunately for each of the A or 5.0 Star grades we report here there must be an opposing low performer.  We really hope to help BND start moving up with their CMS Stars scores.

Four (4) Doctors
Earlier we screened four of our doctors and three of the doctor’s staffs.  The only doctor we addressed in depth was doctor Dobos.

Quick Score Method (QSM)
The quick score method is how most people rate their satisfaction.  Early on we intentionally loaded our co-processor (mind) with four variables and then addressed each helper.  The variables were: A timeframe, scope of our engagements, money applied, and quality of the desired outcome.  In general for quick scoring we are mindful rather than form driven.  About all of those we scored a quick score – an A score indicates we are highly grateful for having them in our lives.

20161209-F: Health Related Engagements
Now using Google we search for Health Related Engagement (HRE).  We still find no other using our term.  We have been using this term since 2012.  We have shared it in some of our posts.  Just let it be known you heard of the HRE concept from us here at MSG.  We intend to develop our HRE concept next quarter (winter 2017).  Briefly, each HRE gets counted.  Next we count the HRE’s associated with our primary provider Brand New Day.  As of today, December 9, 2016 (ItSaFriday!), we have attended forty-three (43) BND driven HREs out of a total of eighty-three (83). Before BND we never knew we needed such an intensive Level Of Care (LOC).  We are very grateful for all of their services and no so for their supports.

Once again we hand deliver individualized Report Cards by hand to our best performers.  As routine we gave TNQ Report to him personally.  Our first TQN Report Card was dated December 13, 2014.  We have already delivered his 2016 Christmas Report Card by hand delivering it to him.  We also gave doctor Boyce an individualized report by hand.  For doctor Boyce we put in some trick material to determine if he reviewed our submission.  If doctor Boyce reviewed our work there was little indication.

Legacy Reports
We are leaving out our legacy reports such as those from 2015 because the trends seem to be flat.  This means few of those evaluated here are vastly improving or have a significant negative slope in performance over time.

New – The Middle Ground
We are working on a Definition for The Middle Ground.  Middle Grounders are not core to the Medical Model (MD & DVM).  Middle Grounders are not Attorneys.  Middle Grounders are otherwise those paid to help such as Social Workers, Therapists, and Program Directors.

The Bethany Rocks Assessment (TBRA)
In the process of fixing a two medical errors (medication errors) earlier this year we got quite frustrated.  Eventually we ironed it out.  In the process of fixing the error we identified another resource.  He name is Bethany the Behavioral Health Nurse.  Because we are so grateful for her helping us we came up with our The Bethany Rocks Assessment.  We will apply it in the future PRN to the middle grounders.  We applied it to an MD (ASK) but it doesn’t seem to reflect her achievement and performance well.  Because TBRA is in development we opted to not include it.

Reference By Name
In the beginning of our Blogging (2012) we ciphered almost all our people’s names.  It was partially due to discomfort about sharing the truth about our providers and associates.  We feared being dropped.  Even though BND corporate has written us twice that this could not happen – “we will not drop you” – we still are a bit anxious when it comes to using real names.  As an indicator for our improvement with our behavioral health we have shared an all time high of real names.  The names that remain ciphered are those likely to cause the biggest conflicts of interest.

Concerns - Corporate or HealthMan (Powered By AVEY)
In 2017 we here at MSG are readying to release our HealthMan bundle.  We just did a Google search for “HealthMan”.  We get four (4) hits about the same operation.  Some of the items that the existing HealthMan addresses are: ” Tariffs, Professional Interests, Commercial Interests, Networking, and Costing Assessment”.  Our MSG HealthMan is significantly different thus we are keeping the name with a modification – HealthMan-AVEY.  One of HealthMan-AVEY’s prime features is a more direct approach to handling concerns.  Rather than go through channels such as filing a grievance we go directly to Blog.  Our Medication Error Complaint (2016) was not solved sufficiently by corporate we had to fix it ourselves.  The benefit was that we now know where Bethany fits into our puzzle.  Yeah!  Bethany Rocks.

Add Resentments
Just today, December 9, 2016, our life coach suggested we add resentment management to this report.  Using our STR (Proprietary) Protocol we are going to take thirty minutes to reflect on those people we resent.  We have three classes: Those that achieved and get to practice stellar doctoring, those that worked against us (knowingly and not knowingly), and those that caused us significant harm (injury).  ItSaFriday and we really can’t be angry with anyone just for today. :o)
